In order for Tower Facilities to utilize your Company as a subcontractor, we are required to obtain certain information.
Sub-Contractor, at its own expense, shall provide and maintain insurance as follows:

Statutory Workers' Compensation coverage for all of its employees, including occupational disease coverage, as required by applicable law, and employer's liability with limits of at least $1,000,000 bodily injury each accident, bodily injury by desease each employee and $1,000,000 bodily injury by desease in the aggregate. The policy shall be endorsed to include "all states" coverage and shall include a waiver of subrogation in favor of Tower Facilities.

Liability Insurance (excluding automobile liability) written on an "occurrence" basis with a combined single limit of at least $1,000,000 per occurrence and aggragate for bodily injury and property damage in a form providing coverage not less than a standard commercial general liability policy including hazards of operation coverage, broad form property damage liability coverage, products/completed operations coverage and broad form contractual coverage with liability limits equal to at least the limits in this Subsection 9.4(b).
The policy shall name Tower Facilities as an additional insured and shall include a waiver of subrogation in favor of Tower Facilities.

Automobile Liability Including protection for automobiles and trucks used by Contractor either on or away from the Stores at which services are provided, with a combine single limit of at least $1,000,000 per occurence for bodily injury and property damage.
The policy shall include coverage for all hired, owned and non-owned, vehicles and also should be endorsed to include a waiver of subrogation in favor of Tower Facilities.

"All Risk" property coverage on all tools and equipment, including rental equipment, used in the execution of the Services, for which Contractor may self-insure, and Contractor hereby waives any right of subrogation against Tower Facilities for any loss or damage to same. If Contractor elects to self-insure, Contractor shall protect Tower Facilities to the same extent as it would if it had an all risk property coverage policy.

Contractor shall furnish Tower Facilities with satisfactory evidence of the required insurance naming Tower Facilities as an additional insured prior to the commencement of the Services. Each policy shall include a provision requiring that at least thirty (30) days prior written notice be given Towr Facilities in the event of cancellation or material change.
